As part of the third Wakefield Community Mental Health Transformation evaluation cycle, Wakefield Mental Health Alliance have created a survey to help us better understand the impact of the Mental Health Community Builders work and help shape the future direction of the roles as a team.

Rachel has worked on so many fantastic projects and supported lots of our members in the local VCSE sector. So we'd really appreciate if you could take the time to feedback your views on the impact of her work and share this survey with any of your team.

The information provided will be anonymised & summarised to be used within evaluation reports and presentations within both West Yorkshire and Wakefield forums. The information provided may also be used by their host organisations (Nova Wakefield District, Age UK, & Young Lives Consortium).
