he Live Well Wakefield team do an incredible job of supporting and empowering people across Wakefield District to take control of their health and wellbeing.
There have been over 12,700 referrals to the service to date and the positive impact social prescribing has on local communities and people is clear to see.
A Primary Care Partner said "I didn’t appreciate the real impacts of social prescribing until I worked closely with Live Well through lockdown and saw the impact their service and social prescribing has. This was in an area where the need was greatest i.e. most deprived, but least engaged and social prescribing changed that for many people.”
