Healthy Hearts Community Hubs open across Wakefield District

An innovative approach to help prevent heart disease has launched across the district.

Healthy Hearts Community Hubs are located in local libraries and community centres rather than at a GP practice.

The community-based approach aims to reduce the number of people suffering from, and dying from, cardiovascular disease by helping people understand more about their cardiovascular risk and supporting them to reduce these risks.

Nearly 1 in 15 adults in Wakefield are registered by their GP as having heart disease. People in the Wakefield District are more likely to die of heart disease before they are 75 than the England average.

Wakefield is one of only four areas in the country, and the only one in Yorkshire, by NHS England to trial the Healthy Hearts model.
