How does Live Well Wakefield work with the VCSE sector?

Let us tell you about their new Core20PLU5 social prescribing project...

It's a great example of the important links between the social wellbeing service, Live Well Wakefield District, and our local Voluntary, Community and Social Enterprise sector.

What is Live Well Wakefield?

Nova and South West Yorkshire Partnership NHS Foundation Trust deliver Live Well Wakefield, which is commissioned by Wakefield Council and Wakefield’s seven Primary Care Networks. The service specialises in holistic, personalised care approaches for Wakefield’s adult residents, delivered through social prescribing and supported self-management projects and interventions.

How does this link to the VCSE sector?

The service is keen to work in partnership with local VCSE organisations to bring sustainable personalised care options to local residents, such as a current Core20PLUS5 funded project led by Live Well Wakefield and delivered in partnership with two local Community Anchors - Eastmoor Community Project and St Mary's Community Centre.

Many adults are impacted by social barriers and non-medical needs that impact their ability to take up and attend health care appointments, and adhere to health care plans, which leads to poor health outcomes. So the project uses the lived experience of Community Anchor volunteers to support clients to attend their health care appointments and overcome social barriers such as independent travel skills, health literacy and social anxiety.

Community Anchors provide support to individuals within the project to engage in activities to create greater connections, and support networks within their local community, whilst Live Well Wakefield supports the individuals to address their wider non-clinical needs via social prescribing.

Referring into the Core20PLUS5 Project

If you have a client who would benefit from this project, please contact Live Well Wakefield.

Referral Criteria

Referrals can be made on the Live Well Wakefield website or through calling the Live Well Wakefield office.

  • 18+
  • History of missed appointments or likely to miss appointment.
  • Referrals must come from a client registered at one of the following practices: Warrengate Medical Centre, Trinity Medical Centre, Eastmoor Health Clinic, Northgate Surgery, Friarwood Surgery, Stuart Road Surgery.
  • Must be able to finance their own travel and self-care.

What appointments?

  • Any hospital appointments at either Pontefract Hospital or Pinderfields Hospital
  • GP appointments
  • Any other healthcare related appointments within your area(dental, ears, eyes etc)

Information to gather for the referral

  • Name, DOB, address, Phone Number
  • GP practice registered to
  • Appointment details: Time, Date, Location
  • Reason for needing support.
  • Preferred method of contact: Phone, Letter, Text Message, Email

How can you connect with Live Well Wakefield?

Live Well Wakefield regularly invites VCSE organisations to speak at their team meetings to help keep the team up to date with local VCSE support available to individuals accessing the service.
