NHS Property Services have worked with Live Well Wakefield to turn unused Primary Care space into Social Prescribing Hubs.

As a result, Live Well Wakefield has a room available at Crofton Health Centre that they would like to offer to VCSE organisations free of charge, for them to run regular social prescribing activities or events from.

The room is also suitable to offer information session from including support around mental health, carers support available within the district and benefits advice and support.

The comfy chairs can be pushed together to form a sofa or split apart to be suitable for activities. The room comes with stackable chairs and foldable tables.

Live Well Wakefield's aim is to develop this space into a vibrant hub within the community for the Crofton population and surrounding areas.

If your organisation could make use of the free space, regularly committing to a slot where social prescribing type support or activities or an upcoming event could take place, and you would like further information, or if you have any thoughts on how the room can be used to best support people’s wellbeing, please contact Shannon (Live Well Coordinator) via email at shannon.simpson@swyt.nhs.uk. They are currently organising room bookings, so please get in touch as soon as possible to ensure they have availability.
