What the fund is about: Nova Wakefield District and South West Yorkshire Partnership NHS Foundation Trust (SWYPFT) deliver Live Well Wakefield, which is commissioned by Wakefield Council and Wakefield’s seven Primary Care Networks. For this round of Live Well Small Grants, funding up to £10,000 (per year for a maximum of 2 years) was available to support:

  • VCSE projects/activities in the east of Wakefield District that meet health and wellbeing related needs, specifically provision for low level mental health support for working age adults (26-50 years of age)
  • Successful organisations will use a standardised wellbeing outcome tool for their projects/activities
